it is a one time purchase. Once your subscription has ran out, you will have to follow the same steps to purchase again

Since the latest update to simplify and improve the billing system, it is a subscription. More information is in that announcement.

You can cancel your subscription at any time from the billing area in your account settings.

Yes.

Cancelling will not affect your access to the subscription for the time you've already paid for. It just stops further billing.
